I, the parent/guardian of the above-named person, acknowledge that the information contained here is correct so far as I know and the person herein described has permission to engage in all activities except as noted. I hereby give permission to the medical personnel selected by Fox Valley Christian Action/Main Baptist Church staff to give over-the-counter medications; to order X-rays, routine tests and treatment; to release any records necessary for insurance purposes; and to provide or arrange necessary related transportation for my child. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the Fox Valley Christian Action/Main Baptist Church staff to secure and administer treatment, including hospitalization, for the person named above. I hereby release Fox Valley Christian Action/Main Baptist Church, its staff, board members and any Fox Valley Christian Action/Main Baptist Church agents from any responsibility in the case of accident, injury or illness during participation in any activities of Fox Valley Christian Action/Main Baptist Church or sponsored programs, on or off-site. I further consent to the use of pictures, video, still frames or voice replication and the first name of the above-named person to be used in articles, other printed materials and/or video advertisements of the above-named person specifically for publicizing the work of Fox Valley Christian Action/Main Baptist Church. I further consent that Fox Valley Christian Action/Main Baptist Church will keep all other matters confidential unless otherwise authorized by the parent/guardian of the above-named person.
MAIN BAPTIST CHURCH NEW GENERATION MINISTRIES PARENT / VOLUNTEER FORM
We ask that you prayerfully consider the position in which you will serve in MBC New Generation Youth Ministries. We ask that our volunteers commit to at least one Sunday per month for 3 consecutive months. If you are not available to serve on your assigned Sunday, we ask that you contact a member of the New Generation Leadership Team as soon as possible so a substitute can be arranged.
